Health Professional’s Report (Form 8)

 This is the document first used by healthcare practitioners to report a patient's work-related injury or illness to the Workplace Safety and Insurance Board (WSIB). Includes detail about the nature of the injury, its potential connection to the workplace, and the patient's treatment plan.

Functional Abilities Form (FAF)

A document that helps employers and health professionals plan a safe return to work for an injured or ill employee. The FAF is used to identify an employee's limitations and work-related responsibilities. 

Worker's report of injury/disease (Form 6)

The Worker’s Report of Injury/Disease form is used to report a work-related injury or illness to the Workplace Safety and Insurance Board (WSIB). It requires the injured worker to provide details about the incident and their medical needs in order to claim compensation benefits. This document serves as formal notification to both your employer and the WSIB about the workplace injury.


Dr. Matthew Brander, D.C.


CHIROPRACTOR

Matthew Brander graduated from the University of Toronto with a Bachelor of Kinesiology. During his time at UofT, he was a Varsity Rower and in his final year was selected as the Lightweight Men’s captain. He is a graduate of CMCC (Canadian Memorial Chiropractic College).

Matt focuses on patient-specific and patient-centered care to address a wide range of conditions including general aches and pains, athletic injuries, arthritic pain, and much more. His treatments incorporate a combination of manual therapy, soft tissue techniques, active rehabilitation, and patient education to optimize health and wellness outcomes. He believes evidence-based care is essential to help his patients achieve their optimal health, function, and well-being.  

Although Matt is new to the Waterloo region, he is becoming an active member of the community. He is a Varsity Rowing Coach with the University of Waterloo Rowing Team and he also serves as the Treasurer for the Cambridge Rowing Club. So there is a good chance if you are out early in the morning you will find him on the Grand River. 

Matt is excited to use his chiropractic, and physical therapy skills to help the Kitchener/Waterloo Region.
